At State of the Heart Care, we are moving beyond the boundaries of traditional hospice and palliative care to provide an integrated approach to end of life care. By providing a wide array of services, we seek to meet clients and patients where they are, providing them with the care they need, when it matters most. Below is a quick overview of our services. Our first program is our enLIGHTEN program. This is a pre-hospice, non-medical program, served mostly by our volunteers. In this division, clients who are newly diagnosed with a chronic or life-limiting illness will have access to educational information regarding their diagnosis, volunteers to assist with housework and errands, and a coordinator who can assist them with accessing valuable community resources.
As an illness progresses, clients from the enLIGHTEN program may need more medical assistance to help manage pain and symptoms while they continue to seek curative treatment. At that point, they may move into our second program–enABLE.
This is what’s traditionally known as palliative care. In this program, patients have access to our physician, nurse practitioners and, if appropriate our social services teams as well. The benefit of this program is that the patient can still be seeking curative treatment and there is no set life expectancy. With this program, we can improve the quality of life for these patients while drastically decreasing hospitalizations and ER visits in the communities we serve.
From the enABLE program, patients may then move into our third program–enCOMPASS. This is the new name for what has traditionally been known as our hospice service. This program continues to provide the vast array of services we have been known for over the last 35 years, but with a new commitment to being more present at the end of life. To the extent we are able; we will make every effort to have an increased presence in the last 7 days of a patient’s life.
Finally, our enCOURAGE program brings new focus to the grief support services we have always offered. We will continue to offer these services free of charge to anyone regardless of their affiliation with State of the Heart. However, by calling attention to these services as their own program, we’re hoping that more people within the community will partake of this great offering.
